Have you been able to obtain all the information you require to apply for the medal? If not, i maybe able to help as I am aware that you served on board in the catering department.
FYI - all QE2 recipients of the medal had it sent to their home address automatically by the Government. The medal was engraved with the crew members name.

26th August 1998 at Invergordon. Drove up for the day to see her again. First time was as she sailed from Le Have in late 1969.
