If I recall correctly the maximum length of a pilot ladder is 9 metres. Any vessel with a greater freeboard has to provide the combination of gangway and ladder. The gangway ( accommodation ladder) has to have means of securing it to the vessels hull at its lower platform.

The large tankers I was on when light ship , they had a short section of pilot ladder connected to cables on a portable air operated winch.
once the pilot was on the ladder, the ladder was then winched up the ship side

The vlcc I sailed on in the 70's had one of those air driven pilot hoists. Could be wrong but believe they have been banned for a number of years now due to them often malfunction and leaving pilots being stranded midway.
The regs are all about accommodation ladder and pilot ladder combination nowadays.
