    So many mistakes by management, the system and other matters.
    Having been an OHS representative in my opinion it is the responsibility of all to ensure the work place, where ever it may be, is safe.
    All works have the responsibility and right to report all matters where there is a concern no matter how trivial management may say it is.

    As has been proven here it was a system, a simple system failure that caused the disaster.
    Somethin that many in management would say was not of great importance had the disaster not occurred.
    But it goes to show that very often it is the small issues that can cause the biggest damage.

    Thanks Fouro - there is certainly more to this than has been let on to. Seems to me the best people to blame are those that cannot answer back.

    Under the circumstances it is easy for management or others to pass the blame onto those who cannot answer.
    A disgusting practice and not the first time such has occurred.

    Any excuse to avoid prosecution, liability or any other form of compensation.

    But as is often the case in such instances there ae not enough who were actually at the seat of the problem able to answer.
    Was there an earlier explosion, was the time correct, so many questions we may never know the truth about.

    Vale those poor souls who perished.

    Given the circumstances and the lack of survivors any conclusions must be to some extent based on suppositions.
    It is one of those events where the full facts will never be known.
    I saw one presentation which made me think the problem began with the lack of control on paperwork.
    Failure of simple systems put in place very often by shoe side persons who have no concept of what life may be like on such a rig, or what could happen if things go wrong.

    Rules and regulations should always be written by those with practical knowledge, not some one with a Uni degree not worth the paper it is written on.

    On BBC4 tonight, there is the story of the deep sea divers on the oil rigs, and of one of them trapped on the bottom with a small air supply. Pretty hazardous work for those guys, kt

    Furo, I have seen a couple of other videos on the disaster.
    All have given similar accounts of what happened that night.
    But who was to blame, hard to place the blame with any one individuals as failure of the systems rather than one individual to my mind is the main cause.
